Altérations Cognitives dans les suites d'un Syndrome Coronarien Aigu : Relations avec l'Affect Étudié par Évaluation Répétitive en Milieu Écologique

Abstract : Introduction: Acute coronary syndrome (ACS) is often associated with cognitive impairment. Anxious and mood syndromes in ACS could explain this cognitive impairment. Ecological momentary assessment (EMA) is an ambulatory monitoring techniques that allows the study of affect and behaviours in daily life. Our aim was to study the factors implicated in cognitive impairment after an ACS. Methods: In an observational, prospective and mono-centric, cohort study, we included subjects four months after an ACS. At inclusion, cognitive testing and EMA was performed. EMA included 5 electronic interviews per day over 7 consecutive days. Six months following inclusion, cognitive testing was replicated. Results: There was evidence of a good validity and feasibility for EMA after analysing the initial samples of 47 subjects for rates of study acceptance, compliance, moods patterns and reactive effects. For the 41 subjects adhering inclusion and follow-up, we found a small minority of impaired subjects in cognitive testing. There was no association between affect levels and being impaired or not for the different cognitive tests. Furthermore, there was no association between affect and changing from impaired to not impaired status. Discussion: EMA is a feasible and valid tool for understanding daily life affect levels and functioning in ACS patients. However, the limited changes in cognitive status in this sample prevented identification of its potential association mood levels. EMA is nonetheless an important new methodology to be expanded to enhance medication adherence and recidivism risk comprehension in this population.
